Pharmacodynamics
Combined analgesic and antispasmodic agent, the combination of the components of which leads to the mutual potentiation of their pharmacological action. Metamizole sodium is a pyrazolone derivative that has analgesic, antipyretic and anti-inflammatory effects. Pitophenone has a direct myotropic antispasmodic effect on smooth muscles (papaverine-like action). Fenpiverinium bromide has an m‑anticholinergic effect and has an additional myotropic antispasmodic effect on smooth muscles.
Indications
For short-term symptomatic treatment of acute pain syndrome of severe and moderate severity with spasms of smooth muscles of internal organs:
– stomach and intestinal colic;
– renal colic in nephrolithiasis;
– spastic biliary dyskinesia;
– algomenorrhea.
Contraindications
Hypersensitivity to the components of the drug, as well as to pyrazolone derivatives, inhibition of bone marrow hematopoiesis, disorders of bone marrow hematopoiesis (for example, due to treatment with cytostatics) or diseases of the hematopoietic system (agranulocytosis, leukopenia, aplastic anemia), severe liver and / or kidney failure, glucose-6 deficiency -phosphate dehydrogenase, tachyarrhythmia, severe angina pectoris, decompensated chronic heart failure, angle-closure glaucoma, prostatic hyperplasia (with clinical manifestations), intestinal obstruction, megacolon, collapse, pregnancy, lactation, atony of the gallbladder and bladder.
For intravenous administration – infancy (up to 1 year) or body weight less than 9 kg.
For intramuscular injection – infancy (up to 3 months) or body weight less than 5 kg.
Carefully
Renal and / or liver failure, bronchial asthma, a tendency to arterial hypotension, hypersensitivity to other non-steroidal anti-inflammatory drugs (NSAIDs) or non-narcotic analgesics, urticaria and / or acute rhinitis, provoked by taking acetylsalicylic acid or other NSAIDs.

Pitophenon hat eine direkte myotrope krampflosende Wirkung auf die glatte Muskulatur (Papaverin-ahnliche Wirkung). Fenpiveriniumbromid wirkt m?anticholinerg und zusatzlich myotrop krampflosend auf die glatte Muskulatur.
Hinweise
Zur kurzfristigen symptomatischen Behandlung des akuten Schmerzsyndroms schwerer und mittelschwerer Schwere mit Krampfen der glatten Muskulatur der inneren Organe:
– Magen- und Darmkoliken;
– Nierenkolik bei Nephrolithiasis;
– spastische Gallendyskinesie;
– Algomenorrhoe.
Kontraindikationen
Uberempfindlichkeit gegen die Bestandteile des Arzneimittels sowie gegen Pyrazolonderivate, Hemmung der Blutbildung im Knochenmark, Storungen der Blutbildung im Knochenmark (z. B. durch Behandlung mit Zytostatika) oder Erkrankungen des blutbildenden Systems (Agranulozytose, Leukopenie, aplastische Anamie) , schweres Leber- und/oder Nierenversagen, Glukose-6-Mangel-Phosphat-Dehydrogenase, Tachyarrhythmie, schwere Angina pectoris, dekompensierte chronische Herzinsuffizienz, Engwinkelglaukom, Prostatahyperplasie (mit klinischen Manifestationen), Darmverschluss, Megakolon, Kollaps, Schwangerschaft, Laktation, Atonie der Gallenblase und Blase.
Zur intravenosen Verabreichung – Sauglingsalter (bis zu 1 Jahr) oder Korpergewicht unter 9 kg.
Zur intramuskularen Injektion – Sauglingsalter (bis zu 3 Monaten) oder Korpergewicht unter 5 kg.
Sorgfaltig
Nieren- und/oder Leberversagen, Asthma bronchiale, Neigung zu arterieller Hypotonie, Uberempfindlichkeit gegen andere nichtsteroidale Antirheumatika (NSAIDs) oder nicht-narkotische Analgetika, Urtikaria und/oder akute Rhinitis, hervorgerufen durch die Einnahme von Acetylsalicylsaure oder anderen NSAIDs .
